Dark circles under the eyes, dimples and tired looks — these scars that don't go away no matter how you sleep, are often not about your lifestyle, but about your anatomy. Undereye filling is a delicate medical aesthetic application that corrects this pitting by injecting hyaluronic acid into the under-eye groove known as a tear trough, reducing the appearance of dark circles and giving a rested expression to the eye area.

Why Does Pitting Under the Eyes Occur?

The tear trough (tear trough) is an anatomical depression that runs from the inner corner of the eye to the cheek area just below the eye socket bone. While it is genetically distinct in some individuals, it deepens for everyone with aging. Causes:

Volume Loss: With age, the fatty tissue around the eyes becomes thinner and slides down. The bone structure also regresses. This dual loss creates a dimpling under the eye that casts a shadow.

Thin skin structure: The skin under the eyes is the thinnest area of the face — about 0.5 mm. This subtlety makes the vascular structure and muscle tissue underneath visible; this is where the dark ring effect comes from.

Genetic predisposition: In some people, the eye socket bone is innately deep or the fat cushion under the eyes is small. In this case, a pronounced groove and dark ring can be seen even at a young age.

Pigmentation: Especially in dark-skinned individuals, the accumulation of melanin in the under-eye area can enhance the appearance of dark rings. In this case, the filler alone may not be enough.

Lifestyle factors: Insomnia, stress, dehydration and allergies can temporarily increase under-eye swelling and dark circles — but they're not the root cause of structural dimming.

Who Does Under Eye Filler Benefit?

This procedure is not the solution to every under-eye complaint. I find that it gives the best result when:

  • There is a pronounced under the eye gutter/pothole those who have (tear trough deformity)
  • Due to volume loss shadow type dark ring the ones that appear
  • Caused by the fact that the vascular structure under the skin is visible purple-blue tone those who have complaints
  • Despite adequate sleep and care those who seem tired all the time

The filler alone may not be enough or may not be suitable if:

  • Prominent under-eye bagging (fatty herniation) — surgical blepharoplasty is more appropriate
  • Dark circles due to pigmentation (melanin) — filler reduces shade but does not change color
  • Very thin and creply skin structure — filling may become visible from below (Tyndall effect)

During the examination, I evaluate your under-eye structure in detail and clearly share whether the filling is the right option for you.

What Products Do I Use?

The under-eye area is one of the most sensitive areas of filler application. Here the choice of products directly affects the result:

Soft, low viscosity HA fillers I prefer. Because the skin under the eyes is very thin, hard or high-viscosity products can appear under the skin, make lumps, or cause a blue discoloration (Tyndall effect).

Products I use:

  • HA fillers specially formulated for under the eyes with low crosslinking density
  • Products with high seating capacity without spreading in a cohesive structure
  • Formulations with integrated lidocaine, increasing the comfort of operation

How is the transaction carried out?

Technical rigor is paramount here, as the under-eye filler is applied to one of the most sensitive areas of the face.

Rating: I assess your under-eye structure sitting and lying down. The depth of the groove, skin thickness, existing fat bagging and the type of dark ring are analyzed.

Anesthesia: Topical anesthetic cream is applied to the under-eye area and waited 15—20 minutes.

Injection: I particularly prefer the use of blunt-tipped cannulas in this area. The cannula significantly reduces the risk of bruising relative to the needle and allows for a more controlled filling distribution. The filler is placed on the bone membrane (periosteum) or at the level of the deep dermis, in very small quantities.

Quantity: Under-eye filler is an area where “az” is beautiful. Usually under each eye 0.3—0.5 ml is sufficient. Excess filling looks bloated and artificial.

Duration: Total transaction 15—20 minutes. The result is visible immediately.

Post Process

  • Slight swelling and tenderness are normal; may last 2—5 days (the eye area heals a little slower than other areas)
  • Bruising can occur; with the use of cannula, the risk is reduced but not zero. If it occurs, it disappears in 5—10 days
  • Do not put pressure on the eye area for the first 24 hours, do not wear heavy glasses
  • Avoid intense exercise, sauna and alcohol consumption for the first 48 hours
  • Do not massage under the eyes
  • The final result is with a complete descent of the swelling 10—14 days becomes clear in

permanence

Undereye filling persistence is longer than other filling areas — average 12—18 monthsIn some patients, it can last up to 24 months. This is due to the fact that the area under the eyes is little mobile, and the filler is slowly absorbed here.

Maintenance sessions are usually scheduled 12—18 months apart. Fewer products than the first application will suffice in each session.

What is the Tyndall Effect?

The most well-known undesirable effect of under-eye filler is the Tyndall effect — it appears as a blue-violet color under the skin if the filler is placed too superficially. To prevent this situation:

  • I apply at the right depth (over the periosteum)
  • I avoid excessive superficial injection in patients with very thin skin structure
  • I start with a conservative amount in the first session
  • I complete it in a control session after 2—3 weeks if necessary

If the Tyndall effect occurs, it can be completely corrected by hyaluronidase.
