Why the Neck Ages First

When we think of facial rejuvenation, the forehead, eye area and cheeks tend to come to mind first. Yet the neck is one of the earliest and most visible areas where aging becomes apparent. After the eyelid, neck skin is the thinnest on the entire body, making it highly susceptible to collagen loss, sun damage and gravitational descent. It is also the area most often overlooked in a daily skincare routine.

Modern aesthetic medicine now offers remarkably effective solutions for neck rejuvenation — many of them without surgery. In this guide I walk through the signs of neck aging, the full spectrum of non-surgical and surgical treatments available, and how I determine the right approach for each patient.

How Neck Aging Develops

Aging in the neck is not a single process; it results from several mechanisms acting together. Choosing the right treatment starts with identifying the right problem.

Platysma bands (vertical cords): The platysma is a broad, thin muscle sheet running from below the jaw to the collarbone. As it weakens with age, its medial borders separate and become visible as two vertical cords that give the neck a distinctly aged look. These bands become more prominent during speech and facial expression, and they also pull the jawline downward, contributing to jowl formation.

Horizontal neck lines (necklace lines): The natural horizontal creases in neck skin gradually deepen into permanent lines. Collagen depletion, chronic dehydration, and the sustained forward-flexion posture from phone and computer use — sometimes called "tech neck" — all accelerate this process.

Skin laxity and crepiness: As collagen and elastin diminish, neck skin thins, loses its resilience and takes on a tissue-paper texture. This is most visible on the lateral neck and the décolletage.

Submental fullness (double chin): Fat accumulation beneath the chin blunts the cervicomental angle, making the face appear heavier and older. Genetics, weight gain and the natural redistribution of facial fat with age all play a role.

1. Neck Rejuvenation with Botulinum Toxin

Botulinum toxin is one of the most widely used and effective tools in non-surgical neck rejuvenation. I use two distinct techniques depending on the clinical picture.

Nefertiti Lift: Named after the Egyptian queen renowned for her elegant neckline, this technique involves precise botulinum toxin injections along the jawline insertion of the platysma and into the vertical bands themselves. By reducing the downward pull of the platysma, the jawline becomes more defined and the neck contour softens. I typically use 20–40 units; the procedure takes around 15–20 minutes and the effect lasts 3–4 months.

The Nefertiti Lift works best in patients whose platysma is actively banding but whose skin has not yet lost significant elasticity. In cases of advanced skin laxity, botulinum toxin alone may not be sufficient.

Microbotox (Mesobotox): Here, a very dilute solution of botulinum toxin is delivered through multiple micro-injections into the superficial dermis of the neck. The goal is not full muscle paralysis but a selective weakening of superficial muscle fibres. The result is smoother fine lines, improved skin texture and a subtle tightening effect across the neck. While the Nefertiti Lift targets the platysma bands directly, microbotox improves overall skin quality — the two techniques complement each other well.

2. Neck Rejuvenation with Biostimulators

Biostimulators are injectable products that improve skin quality over time by triggering the body's own collagen production. Two products are particularly well suited to the neck.

Hyperdilute Radiesse: Radiesse (calcium hydroxylapatite) is diluted with normal saline and lidocaine and distributed across the neck and décolletage in a superficial injection plane. In this formulation the objective is not volume addition but rather a structural improvement in skin thickness, firmness and texture through collagen and elastin neosynthesis. Hyperdilute Radiesse produces excellent results for neck crepiness, skin thinning and mild laxity. The effect typically lasts 12–18 months.

Sculptra: Sculptra, based on poly-L-lactic acid (PLLA), can also be used in the neck to stimulate gradual collagen remodelling. Results may persist for up to two years. However, the neck carries a higher risk of nodule formation with PLLA, making practitioner experience particularly important.

The combination of botulinum toxin and a biostimulator forms one of the most powerful non-surgical protocols in neck rejuvenation: botulinum toxin relaxes the platysma bands while hyperdilute Radiesse rebuilds the skin's structural framework from within.

3. Mesotherapy and Skin-Quality Treatments

Because neck skin is thin and delicate, superficial treatments that enhance hydration and stimulate repair play an important supporting role.

Mesotherapy: Microinjections deliver cocktails of hyaluronic acid, vitamins, amino acids and peptides directly into the neck dermis. This hydrates and nourishes the skin while promoting collagen synthesis. A typical course consists of 3–6 sessions at weekly or fortnightly intervals.

Profhilo: Profhilo contains a high concentration of pure hyaluronic acid and is injected at only five anatomical points using the BAP (Bio Aesthetic Points) technique. It spreads beneath the skin to deliver deep hydration and stimulate both collagen and elastin production. It is highly effective for overall skin-quality improvement in the neck and décolletage. Treatment begins with two sessions spaced four weeks apart.

PRP (Platelet-Rich Plasma): Platelet-rich plasma prepared from the patient's own blood provides concentrated growth factors that support tissue regeneration. I often combine PRP with mesotherapy or microneedling to amplify skin-quality results in the neck.

4. Filler-Based Corrections

In certain neck deformities, targeted filler injections can provide precise correction.

Horizontal neck lines: Deep horizontal creases can be softened by injecting a thin, low-viscosity hyaluronic acid filler beneath the line. Because neck skin is thin, soft formulations are essential and the injection must remain at the correct depth to avoid the Tyndall effect (a bluish discolouration visible through transparent skin).

Jawline and submental zone: Radiesse or hyaluronic acid fillers placed along the jawline and at the cervicomental angle can sharpen definition and improve the overall neck–jaw transition. Although technically a jawline treatment, this significantly enhances the appearance of the neck as a whole.

5. Thread Lift

Absorbable threads made from PDO (polydioxanone) or PLLA are inserted beneath the neck skin to produce a mechanical lifting effect. As the threads dissolve over time they also stimulate collagen production in the surrounding tissue. Thread lifting can be considered as a non-surgical alternative for mild to moderate neck sagging. However, the results are more limited than a surgical neck lift and typically last 12–18 months.

The effectiveness of thread lifting in the neck depends heavily on correct patient selection and practitioner experience. In patients with significant skin laxity, threads alone may not deliver an adequate result.

6. Surgical Neck Lift (Necklift)

When skin laxity, prominent platysma bands or submental fullness have progressed beyond what non-surgical methods can meaningfully correct, a surgical neck lift is the most effective and lasting solution.

The procedure typically involves small incisions behind the ear through which excess skin is removed, the platysma muscle is tightened and rejoined at the midline (platysmaplasty), and — when indicated — liposuction is performed in the submental area. Surgery can be carried out under local anaesthesia with sedation or under general anaesthesia and usually lasts 1.5–2.5 hours.

A surgical neck lift can be performed as a standalone procedure or in combination with a facelift. Results are long-lasting: depending on skin quality and lifestyle factors, the improvement can be maintained for 7–10 years.

Which Method Is Right for You?

There is no single correct answer in neck rejuvenation — the right plan must be tailored to each patient's anatomy, aging pattern and expectations.

Prominent vertical platysma bands: The Nefertiti Lift (botulinum toxin) is my first choice. When skin elasticity is preserved, it can be remarkably effective on its own, particularly in younger patients.

Skin thinning, crepiness and loss of texture: Skin-quality restoration with hyperdilute Radiesse or Profhilo. Mesotherapy can be added as a supportive treatment.

Horizontal neck lines: Targeted correction with a fine hyaluronic acid filler. Combining this with microbotox helps slow line recurrence.

Mild to moderate sagging: A botulinum toxin plus biostimulator protocol. Thread lifting can be added if further mechanical support is needed.

Advanced sagging, platysma laxity or submental fullness: A surgical neck lift is the appropriate treatment. Non-surgical methods alone are insufficient at this stage.

Combined approach: In most patients, the best outcome is achieved by layering more than one modality. For example, platysma bands are first controlled with botulinum toxin, then skin quality is rebuilt with hyperdilute Radiesse — producing a result that neither treatment could achieve alone.

Frequently Asked Questions

Are neck rejuvenation treatments painful?For injection-based treatments, topical anaesthetic cream is applied beforehand and most patients tolerate the procedure comfortably. Surgical neck lifting is performed under anaesthesia.

How long do neck rejuvenation results last?Botulinum toxin lasts 3–4 months, hyperdilute Radiesse 12–18 months, Profhilo 6–9 months, thread lifting 12–18 months, and surgical neck lifting 7–10 years. Duration also depends on lifestyle factors and skin quality.

When can I return to normal activities?After injection treatments, most patients resume daily activities on the same day. Surgical neck lifting involves a social recovery period of approximately 1–2 weeks.

Can a facelift be combined with neck rejuvenation?Yes. A surgical neck lift is most often planned alongside a facelift for a harmonious result. Non-surgical neck and face treatments can also be combined in the same session.

At what age should neck rejuvenation be considered?Signs of neck aging can appear from the early 30s depending on genetics and lifestyle. Preventive mesotherapy and targeted skincare are appropriate at younger ages; botulinum toxin and biostimulators are typically considered from 35–40; and surgical options are usually evaluated in patients over 50.
