Contrary to popular belief, lip filler is not a simple procedure anyone can perform — it requires an injector with genuine anatomical knowledge, training, and experience. Complication rates are generally low when performed correctly, but the risks rise sharply when the procedure is carried out by unqualified providers.

After the injector's training and experience, the second most important factor is the quality of the filler itself. The hyaluronic acid used must be highly purified through advanced manufacturing technology; the purer it is, the lower the risk of reaction or granuloma, and the more predictable the longevity of the result.

How to Achieve a Natural-Looking Lip Filler

The biggest fear for anyone considering lip filler is ending up with an overfilled, "duck lip" look. In reality, a well-executed lip filler doesn't cause that look at all — nearly every overfilled example you notice is the result of poor planning, not the treatment itself.

G′ Value

A filler's G′ value reflects its firmness and elasticity. I avoid high-firmness products for the lips — low to medium firmness fillers feel much closer to natural lip tissue.

Cohesivity

Cohesivity describes how strongly the hyaluronic acid molecules bind together. In the constantly moving lip area, highly cohesive fillers integrate smoothly with surrounding tissue without clumping.

In short: a natural-looking lip filler calls for a low-to-medium firmness, highly cohesive filler.

The recently popular "baby lips" look is really built on the same principle: a minimal amount of soft filler, used to make the lips look hydrated and healthy rather than dramatically larger.

Can Lip Filler Be Dissolved?

Yes. Hyaluronic acid based lip fillers can be dissolved with the enzyme hyaluronidase when needed. The enzyme breaks down the hyaluronic acid so it clears from the tissue quickly. It's a safe option for cases of dissatisfaction, asymmetry, or the rare complication — and one of the biggest advantages HA fillers have over permanent alternatives.

Why I Use Ultrasound for Lip Filler

Ultrasound use in lip filler has become far more common with the rise of portable, handheld devices. Its main benefit: I can see the blood vessels, muscle and mucosal layers, and any granulomas from previous treatments in real time, and guide the needle accordingly.

Two labial arteries run the length of the upper and lower lip. The most serious complications in lip filler occur when product is injected directly into, or too close to, these vessels — causing occlusion or external compression, which can compromise tissue perfusion.

  • Avoiding injections that are too deep or too superficial, with depth carefully calibrated
  • Injecting slowly and in small increments, rather than large volumes at once, while monitoring circulation
  • Aspirating before injecting to check for blood in the syringe — if present, repositioning to a different point
  • Keeping the patient at the practice for observation after treatment before they leave

Since the vessels run parallel to the lip axis, I favor injecting perpendicular to it — the basis of the "Russian Lips" technique — which I also believe produces a better lifting effect. That's why ultrasound assessment before every lip filler session is standard in my practice.

Russian Technique

Injections are placed perpendicular to the lip axis, aiming for an upward lifting effect without adding forward projection.

French Technique

Defines and enhances the lip border while adding volume with forward projection.

An experienced injector can predict which direction produces which effect. In my experience, most patients get the most satisfying result from a combination of both techniques — for example, vertical (Russian) injections in the upper central lip paired with horizontal (French) injections in the lower central lip.

How Much Filler Do You Need? 0.5ml, 1ml or 2ml

Lip fillers almost always come in 1 ml syringes, since the average treatment uses between 0.5 ml and 1 ml. Some patients need up to 2 ml. An experienced injector can usually estimate the right amount during consultation; I still schedule a follow-up 7–10 days after treatment to check the result.
